Current Patients can use our Patient Portal
A new patient packet will need to be completed at your initial appointment unless printed and completed ahead of time. If you don’t complete it before your visit, please plan on arriving 15 minutes prior to your appointment.
If you had a sleep study from another location or have any other pertinent records that should be available for the provider at your appointment, please complete a Release of Medical Information form and send it to that facility or provider so that these records can be received in our office prior to your appointment. There is a copy of the form in the New Patient Packet.
Please arrive at least 15 minutes prior to your appointment if you have not already filled out the First Appointment Documents.
A list of medications you are currently taking.
A method of payment for your visit. Co-pays, co-insurance or deductibles are due at the time of visit, prior to seeing the provider.
Please arrive at your appointment on time. SleepMS requires 24 hours’ notice for any cancellation; otherwise, you will be charged a $50 no-show fee for office visits and $100 no-show fee for sleep studies.
If you are later than 15 minutes for your appointment, we will have to reschedule your appointment.
